COMLEX Level 1 · COMLEX-USA Level 1

The science, with your hands on it. OMM is not an afterthought.

The foundational biomedical sciences inside clinical presentations, the way NBOME asks them — and osteopathic principles and practice as a domain of its own: somatic dysfunction, Fryette, viscerosomatic reflexes, Chapman points and the OMT that treats them.

One in six questions is OPP and OMT.

COMLEX Level 1 is 320 questions in eight 40-item sections, pass/fail. Every question here carries its domain, so you see whether the gap is a science or the osteopathic thread through all of them.

The real paper is 320 questions. Our practice papers are 300 items. Every item is tagged to nine discipline domains with OPP/OMT, so your score is reported area by area. 3,096 original questions in all, counted once.

2Explanations

Every wrong answer gets its own reason

Each explanation says why the right answer is right and why each distractor fails — the exact line you would want if you had picked it. Every item is checked blind by a second AI model that never sees the key. This one is a real item from the On-level 1 form.

ON-LEVEL 1PATHOLOGYMEDIUM FORM

A 60-year-old smoker has hematuria, flank pain, and a flank mass; a renal tumor shows clear cells with abundant lipid/glycogen and invades the renal vein.

Which malignancy and genetic association is this?

A
Wilms tumor with WT1 mutationWilms tumor occurs in children, not a 60-year-old with clear cells
B
Clear cell renal cell carcinoma with VHL gene alterationsClear cell renal cell carcinoma, the most common renal malignancy in adults (smoking a risk factor), arises from proximal tubule epithelium, shows clear cytoplasm (lipid/glycogen), tends to invade the renal vein, and is linked to VHL gene loss (sporadic and in von Hippel-Lindau disease).
C
Transitional cell carcinoma of the renal pelvisTransitional cell carcinoma arises from the urothelium, not from clear cells with lipid/glycogen
D
Angiomyolipoma associated with tuberous sclerosisAngiomyolipoma is a benign tumor of vessels, muscle, and fat, not a clear cell carcinoma
E
Renal oncocytoma, a benign tumor with a central scarRenal oncocytoma is a benign eosinophilic tumor with a central scar, not the VHL-associated clear cell carcinoma described.
3Axon Live

A tutor who asks what you do first — and why

Say what you are stuck on, out loud. Axon teaches the section in the voice of COMLEX-USA Level 1 (NBOME), draws the reasoning on a board as it speaks, and quizzes you on it before it moves on. It knows your syllabus, your weak spots and where you left off.

  • Teach me, Quiz me, I’ll listen, Why I missed it, Cram
  • Fifteen free minutes to meet it, once; top up in packs after that
  • Every session leaves a recap you can reopen

Questions

Before you start

Are the practice exams the real length?

No, and we say so. COMLEX Level 1 is 320 questions in eight sections of 40, two 4-hour sessions in one day. Our twelve practice papers are 300 items each; the full mock draws 320 from all of them and runs the eight hours as one sitting.

Is there a score?

NBOME has reported Level 1 as pass or fail since May 2022, with a Formative Performance Profile beside it. The standard behind the word is not released. SynkMind’s readiness estimate is its own, not NBOME’s, and its line is marked unverified.

How is OMM taught?

In its own lecture — the tenets, somatic dysfunction, Fryette mechanics, viscerosomatic reflexes, Chapman points, and muscle energy, HVLA, counterstrain, myofascial, lymphatic and cranial techniques with their contraindications — and in about one question in six. Techniques are described in words; there are no technique videos yet.

Are these real exam questions?

No. Every item is original, written to the public content outline and checked blind by a second AI model that never sees the key. Nothing is copied from a published form, and we are not affiliated with any examining board.

How does the free day work?
